Q:My blood pressure is good at home, I measure every day, and my primary care has also documented that my blood pressure is within normal limit, so am good to certify for my FMCSA certificate?
A:Not necessarily, FMCSA states that ONLY THE BLOOD PRESSURE readings obtained by the examiner during THE INITIAL and FOLLOW-UP examination can be used for certification decision.
Q:My blood pressure is greater than 139/89 mmHg, do you have to take a second measurement?
A:Yes, the examiner is required to take a second measurement later on during your INITIAL or FOLLOW-UP EXAMINATION.
Q:During my examination, it is determined that I have ELEVATED BLOOD PRESSURE. Is the VIZAVEE DOT examiner going to treat my blood pressure.
A:No, the treating clinician, your PRIMARY CARE, is the one responsible for confirming the diagnosis, prescribing treatment, and determining the effectiveness of the treatment.
Q:I see that VIZAVEE URGENT CARE see SELF PAY, and it has PRIMARY CARE SERVICES. Can I be treated by VIZAVEE for my ELEVATED BLOOD PRESSURE?
A: There are three scenarios for you to be treated for ELEVATED BLOOD PRESSURE at VIZAVEE URGENT & PRIMARY CARE
A1:OPTION-ONE: You have an COMMERCIAL INSURANCE that is accepted by VIZAVEE URGENT & PRIMARY CARE and you want to ESTABLISH PRIMARY CARE. For that, you will have to register and sign up with our PRIMARY CARE SERVICES.
A2:OPTION-TWO: You have an COMMERCIAL INSURANCE that is accepted by VIZAVEE URGENT & PRIMARY CARE and you want to be seen under our URGENT CARE SERVICES to treat your ELEVATED BLOOD PRESSURE. For that, you will have to register your insurance and make an appointment to be treated.
A3:OPTION-THREE: You DO NOT HAVE COMMERCIAL INSURANCE and you want to use our URGENT CARE SELF-PAY SERVICES to treat your ELEVATED BLOOD PRESSURE. For that, you will have to pay $150 for which you be evaluated and treated for the ELEVATED BLOOD PRESSURE examination. Our SELF-PAY services allows you to return for examination for 14-DAYS after your first visit for NO ADDITIONAL FEES, it does not include labs, ekg, or any other required testing or treatment. After day 14, you will have to pay another SELF-PAY VISIT fee.
Q:What is the difference from URGENT CARE SERVICES and PRIMARY CARE SERVICES?
A: As an urgent care patient, you are not entitled to medication refills, or phone advice. You will be treated for each and every calendar day you need services, and all evaluation will require a re-visit and a face-to-face examination.
Q:How often do I have to be seen in order to treat my ELEVATED BLOOD PRESSURE?
A:In order to establish a GOOD RECORD, you will be expected to be seen NO LESS than every 4-MONTH or 3 times per year. This will help us to keep a good record for FMCSA certification and / or re-certification
Q:Why did you give me a 3-MONTH certification?
A:According to FMCSA 49 CFR 391.43(f) “…An individual diagnosed with STAGE 2 (BP 160/100 to 179/109) should be treated and a ONE-TIME CERTIFICATE for 3-MONTH certification can be issued”. The VIZAVEE DOT EXAMINER has NO OTHER OPTION on this matter.
Q: My blood pressure is SUPER HIGH, greater than 180/110, what can I do?
A: We CANNOT CERTIFY you with this HIGH BLOOD PRESSURE. Our best advice is for you to find a PRIMARY CARE for your blood pressure management. Alternatively, you can join our PRIMARY CARE and or SELF PAY services, and we will assist you in controlling your blood pressure.
Q: I am taking a MEDICATION that is used for BLOOD PRESSURE, but I am using it for other purpose than BLOOD PRESSURE management. I do not want to have a HYPERTENSION diagnosis, what do I do?
A: There are many medications that have multiple uses, and we will be able to determine that during our DOT EXAMINATION. It is best that you reveal any and all medication you are currently taking. When antihypertensive medication is used to treat an underlying condition (other medical problems) we MUST BE able to evaluate your tolerance to such medications.
Q: I got GREAT BLOOD PRESSURE(less than 139/89 mmHg) at home, and I am taking BLOOD PRESSURE medication, can I get a 2-YEAR CERTIFICATION?
A: NO, we can only certify a patient taking blood pressure medication for 1-YEAR.
Q: I went to get my DOT CERTIFICATE and the VIZAVEE DOT EXAMINER gave me a 3-MONTH certificate. My blood pressure was 133/80 mm Hg, and I am taking my medication FAITHFULLY. What do I have to do to renewal for one-year?
A:You must get your blood pressure to LESS THAN 130 systolic (TOP NUMBER) and LESS THAN 80 (BOTTOM NUMBER). You are classified STAGE-1 HYPERTENSION, and you were likely given a ONE-YEAR period opportunity to get your BLOOD PRESSURE LOWERED. After that, you be given a 3-MONTH and your pressure has to be reduced to LESS THAN 130 mm Hg.
Q: I came back from my ONE-TIME 3-MONTH CERTIFICATION and the DOT VIZAVEE EXAMINER certified for 9-MONTH ONLY. I thought I was entitled to an ONE-YEAR CERTIFICATE, what can and do to fix that?
A: The elapsed time of the ONE-TIME, 3-MONTH CERTIFICATE is included in the ANNUAL CERTIFICATION interval; it is not an extension to the certification period. This means that we will use the DATE ON THE ONE-TIME, 3-MONTH CERTIFICATE to calculate the medical certificate expiration date. We CANNOT use the date of the FOLLOW-UP examination (YOUR SECOND VISIT) to check your adequate/acceptable blood pressure.
Q:My blood pressure used to be SUPER HIGH, now it has IMPROVED, but still elevated, greater than 139 mm Hg. I have already had 3-MONTH CERTIFICATION, can I get another 3-MONTH evaluation?
A: There has bee a good deal of confusion whether the “one 3-MONTH CERTIFICATE” is a LIFETIME or SINGLE EPISODE GUIDANCE. A statement from FMCSA indicated that this is A GUIDANCE and would not prohibit an examiner from issuing another 3-month certification ” if the examiner believes the medication condition does not prohibit but does require more frequent monitoring.”
Q:I am now taking medication and my blood pressure was SUPER HIGH, now it is great and under 140/90 mm Hg. However, the VIZAVEE DOT Examiner only gave me a 6-MONTH CERTIFICATION, why?
A. Your HYPERTENSION was classified as STAGE-3, and for that you are required to certify EVERY 6-MONTH.
Q: I am taking 3-MEDICATIONS for BLOOD PRESSURE. Can I get a 1-YEAR CERTIFICATION?
A: No, if you are taking three mediation you are likely to suffer from REFRACTORY HYPERTENSION and you must certify EVERY 6-MONTH
Q: My Pulse OXIMETRY is less than 92 %, am I going to be able to obtain my CERTIFICATION?
A: No
Q:I have ASTHMA, and I am coughing a lot. Is this going to be a PROBLEM for me to get my DOT CERTIFICATE?
A:You may be asked to have a PULMONARY FUNCTION test. That is an in-office test in which you will blow in an instrument and if you are deemed to have UNCONTROLLED ASTHMA you will have to further test it. We STRONGLY ADVISED you to BRING IN your ALBUTEROL INHALER if you have asthma.
Q: I recently fell off my truck and developed a PNEUMOTHORAX, is this going to restrict me from obtaining my DOT CERTIFICATE in any way?
A: You will have to provide a CHEST X-RAY demonstrating the pneumothorax COMPLETE RESOLUTION. You will NOT BE ABLE to obtain a DOT CERTIFICATE if you have experience 2 or more SPONTANEOUS PNEUMOTHORAX. For that, you will have a SPECIALIST (LUNG DOCTOR=PULMONOLOGIST) clearance.
Q: My friend was doing his DOT at VIZAVEE URGENT CARE and they measure her neck, what was that for?
A: Drivers with NECK-SIZE of 17 inches for males and 15.5 inches for females are more likely to suffer from OBSTRUCTIVE SLEEP APNEA. That is a condition that may restrict your ability to obtain your DOT CERTIFICATE.

Q: I have a history of SEIZURE disorder as a CHILD. Do I qualify to obtain a DOT CERTIFICATE?
A: Yes, a driver with a history of seizures limited to CHILDHOOD FEBRILE SEIZURES an be certified for 2-YEARS.
Q:I got really dizzy, and my doctor treated me for VERTIGO? How long do I have to WAIT till I get my DOT Certificate?
A:The waiting period is a minimum of 2-MONTHS with NO SYMPTOMS and NO MEDICATION/TREATMENT.
Q:I am an INSULIN-DEPENDENT-DIABETIC, can I get a DOT certifiate?
A:Yes, but first you must obtain and have your PRIMARY CARE FILL OUT the INSULIN-TREATED-DIABETIC-MEALLUS form, MCSA-5870. In it, your primary care must attest that you maintain a STABLE and ADEQUATE insulin regiment and proper diabetes control.
Q:What is the longest time an INSULIN-DEPENDENT-DIABETIC can be certified?
A:The driver MUST PROVIDE a 3-MONTHS of self-monitoring electronic blood glucose record prior to deceiving a 12-MONTH-CERTIFICATION.
