FLORIDA SCHOOL ENTRY EXAM

School physicals are available all year round, EVERY DAY, EVERY HOLIDAY & EVERY WEEKEND. In order to expedite your child’s SCHOOL PHYSICAL, please have the proper paper work filled out. Please click FLORIDA SCHOOL ENTRY EXAM to be directed to the Florida School Entry exam form DH3040-CHP-07/2013. Parent or Guardian must fill out page -1- with the child’s medical history, sign and date.

QUESTIONS & ANSWERS

Q: Can I make an appointment for my child’s school physical?

A: NO, we make NO APPOINTMENT for SCHOOL PHYSICALS. All of our locations will take physicals on a walk in basis, first come, first served, any time during our operating hours.

Q: Can my friend bring my child to have this School Physical?

A: NO, the parent or guardian will have to be present with a VALID PHOTO ID, preferably a DRIVER’S LICENSE, a MILITARY ID, PASSPORT or CONCEALED CARRY PERMIT are also acceptable. There are NO EXCEPTIONS to this rule.

Q: Can I fill out a MINOR CONSENT FORM and allow a family member to bring my child for the SCHOOL PHYSICAL EXAM?

A: Yes, a parent may fill out the form allowing a family member to bring in the child to have the SCHOOL PHYSICAL EXAMINATION. That means a FAMILY MEMBER, NO FRIENDS, NEIGHBORS etc.

Q: How much does all of this cost?

A: As a service to our community, our SCHOOL PHYSICAL COSTS 25 dollars. Most Minute Clinics, Urgent Care Centers and Primary Care offices charge anywhere from $50-150 without insurance. We provide this service all year round. Anytime or Day during our hours of operations in any of our facilities.

Q: I have to get a HIGH SCHOOL SPORT PHYSICAL and SCHOOL ENTRY EXAM. Are these two exams the same?

A: NO, they are two different exams and each one will cost 25 dollars. HIGH SCHOOL SPORTS physical are documented yet in another form. Click on FLORIDA HIGH SCHOOL ATHLETIC ASSOCIATION to download the form if you need a HIGH SCHOOL CLEARANCE for sports activity.
